Dr. Hurmatov provides in-clinic general vaccination and flu seasonal vaccination.
All vaccinations are done only during the properly scheduled appointment time.

We do not provide Covid -19 vaccination, however, our next-door pharmacy Pharma-Star does this service including adult and children Covid and Flu vaccination.
Please call the pharmacy at 905-984-4111 to schedule an appointment for your Covid or Flu vaccinations.
If you bring your child or children for their vaccination to our clinic please, remember, this is your responsibility to follow the Public Health Vaccination Schedule and book an appointment for your child for the next vaccination.

Please fill in the part with your information, print the form call our office, schedule an appointment, and bring the form with you to your appointment for the doctor to complete the form.
UNINSURED SERVICES:
The Ontario Health Insurance Plan (OHIP) covers not all services provided by the medical clinic. You can find information here about What OHIP covers.
The other services must be paid for privately by patients, their insurance companies, or lawyers. These services and charges are regulated by CPSO and OMA.
All physicians use the Ontario Medical Association Guide to Uninsured Services to create the price list of their uninsured services.
